Shots 4 Tots began in 1992 with the goal of increasing immunization rates among Lucas County children. The immunization rate has increased from 33% in 1992 to 73% for Toledo children and from 43% to 82% for suburban children in Lucas County.
Shots 4 Tots is an immunization program for children from birth through 18 years of age. Our professional nurses provide immunizations to children at 13 community locations each month.
All Shots 4 Tots n Teens patients MUST present their insurance card and a HARD COPY of child’s immunization record, and parent’s I.D. at each visit.
Shots 4 Tots and Teens is here to serve your family during the Covid-19 pandemic. It is still important to protect your child with the recommended vaccines. We are holding clinics at our downtown location by appointment only: Call to schedule an appointment and beat the Back to School rush! For appointments call us at 419-213-2013. If you have questions about your child’s vaccines, please call the nurse at 419-213-4121. Face coverings are required for adults and children 5 years old and up, unless you have a medical condition that prevents it.
